Improving the flow of data between services enables better quality care and support for individuals through their journey. Person Centred Software has the capability to join up care services digitally with health services both locally and nationally. As the Head of Healthcare Engagement I am here to support building partnerships throughout the health and care pathways to facilitate sharing of care records on a local, regional and National level.
