After witnessing how "modern" healthcare failed my grandfather after suffering through multiple chronic conditions between 2012-2015 I forewent medical school to pursue a greater vision and passion of empowering people living with chronic conditions anytime, anywhere.

Healthcare in the US is more like disease care where we treat symptoms of chronic conditions instead of preventing or managing them proactively with lifestyle and behavioral changes. HealthSnap exists to make virtual chronic disease management simple and accessible through a combination of remote patient monitoring, chronic care management, care coordination tools, and clinical decision support centered around each patient's unique needs.
